Outpatient Psychiatric Care for Adults

Harmonia Precision Mental Health provides psychiatric evaluation, medication management, and treatment optimization for adults in San Diego and through telehealth across California. Care is individualized to the clinical picture, treatment history, medication response, and co-occurring psychiatric needs.

Psychiatric Evaluation

A psychiatric evaluation looks beyond a symptom checklist to understand the broader clinical picture. Initial consultations examine the primary concerns prompting care, the trajectory of current symptoms, and their functional impact on daily life, work, and relationships.

An evaluation explores:

  • Current psychiatric symptoms and functional concerns
  • Psychiatric and relevant medical history
  • Prior diagnoses and previous treatment response
  • Medication effectiveness, side-effect burden, and tolerability
  • Sleep patterns and physical health contributors
  • Diagnostic uncertainty, overlapping symptoms, or substance use when clinically relevant

The purpose of an evaluation is not simply to assign a diagnostic label, but to develop a clearer clinical understanding that can guide treatment recommendations. This thorough diagnostic review helps clarify underlying symptom drivers when prior treatment has yielded incomplete relief, persistent side effects, diagnostic ambiguity, or an increasingly complex medication regimen.

Medication Management & Optimization

Ongoing medication management extends far beyond routine prescription refills. Follow-up care involves regular review of symptom response, overall medication effectiveness, side-effect burden, patient tolerability, treatment adherence, and dose appropriateness.

During clinical follow-ups, we assess potential drug interactions, evaluate whether a regimen has accumulated unnecessary complexity over time, and verify that each prescribed agent continues to perform a distinct therapeutic role. When adjustments are indicated, changes are made step-by-step and evaluated over time, rather than initiating multiple concurrent alterations when clinically feasible.

Harmonia practices under the clinical philosophy of a Minimum Effective Regimen. This principle does not advocate reducing medication simply to minimize prescription count; rather, it aims to establish a treatment plan that safely and effectively manages symptoms without unnecessary pharmacological complexity.

Explore Our Approach

Adult ADHD Treatment

Adult ADHD evaluation and medication management focus on core executive challenges, including sustained attention, concentration, organization, task initiation and completion, impulsivity, and emotional regulation when clinically relevant. Assessment examines long-standing symptom patterns, prior treatment experiences, medication response, sleep quality, and co-occurring mood concerns.

Adult ADHD is not always experienced in isolation. Evaluation and treatment consider co-occurring psychiatric conditions when present so the treatment plan reflects the broader clinical picture.

Attention difficulties frequently interact with or stem from co-occurring anxiety, depression, sleep disruption, or mood disorders. Diagnostic clarification helps distinguish primary executive dysfunction from secondary cognitive difficulties caused by mood or sleep disturbances, ensuring that treatment recommendations prioritize clinical accuracy, safety, and long-term tolerability.

Anxiety & Depression

Harmonia offers outpatient psychiatric evaluation and medication management for generalized anxiety, persistent worry, panic symptoms, intrusive rumination, depressive episodes, reduced motivation, and incomplete response to prior antidepressant trials.

Treatment planning begins by assessing previous medication trials, tolerability history, and symptom patterns over time. Because anxiety and depressive symptoms regularly overlap with adult ADHD, sleep disturbances, trauma-related distress, or mood disorders, evaluation focuses on identifying these relationships to build a targeted, manageable medication plan.

Bipolar Disorder

Medication-focused psychiatric care for bipolar spectrum disorders prioritizes longitudinal mood stability, treatment response, side-effect monitoring, and ongoing tolerability. Evaluation considers history of depressive episodes, elevated or activated mood states, sleep alterations, and seasonal or stress-related recurrence patterns.

Care involves evaluating prior medication trials, adjusting maintenance treatment when clinically indicated, and addressing co-occurring anxiety, attention symptoms, sleep disruption, or substance use to support long-term stability while minimizing medication burden.

Insomnia & Sleep Disturbances

Sleep problems can manifest as difficulty falling asleep, middle-of-the-night awakening, or unrefreshing sleep. These disturbances frequently occur alongside anxiety, depression, ADHD, or mood disorders, or may present as a medication side effect.

Psychiatric evaluation identifies underlying clinical and pharmacological contributors to sleep disruption. The objective is to clarify these factors before determining treatment adjustments that support restorative sleep while avoiding unnecessary medication burden.

Substance Use Disorders

Harmonia provides outpatient psychiatric evaluation and medication management for substance use concerns alongside co-occurring psychiatric conditions.

Substance use and psychiatric symptoms can influence one another. Care may include evaluation and treatment of anxiety, depression, ADHD, mood symptoms, sleep concerns, and other psychiatric conditions that occur alongside substance use.

Care involves evaluating substance use patterns, reviewing prior psychiatric treatment, and integrating medication management with your broader recovery goals. (Harmonia is an outpatient psychiatric practice and does not provide inpatient detoxification, residential treatment, or emergency withdrawal management.)

Medication-Assisted Treatment (MAT)

Medication-assisted treatment may be incorporated into outpatient psychiatric care when clinically indicated. This includes evaluation for buprenorphine-based treatment options such as Suboxone or Sublocade as part of a comprehensive psychiatric management plan.

MAT services include:

  • Assessment of substance use history and clinical appropriateness for MAT
  • Medication initiation or continuation when clinically indicated
  • Psychiatric treatment of co-occurring mental health conditions
  • Ongoing safety monitoring, treatment reviews, and laboratory testing when clinically indicated
