Clinical Pathways

Care intensity matched to your clinical needs — from structured foundational treatment to periods requiring closer calibration.

Track 1: Precision Foundational Care Structured evaluation and longitudinal medication care, delivered at clinically appropriate intervals for patients who do not require high-density optimization.

Comprehensive Evaluation

  • Thorough diagnostic assessment with deliberate, evidence-aligned prescribing.

Defined Follow-up

  • Defined follow-up intervals appropriate for stable treatment trajectories or early-stage optimization.

Accessibility

  • Select in-network insurance plans are accepted for foundational care.

Track 2: The Precision Arc (90-Day Phase)
A defined escalation pathway for periods when standard visit intervals are clinically insufficient.

The Clinical Indicator

  • When medication response remains unstable, highly sensitive, or incompletely consolidated, standard visit spacing may not provide adequate clinical control.

The Intervention

  • In these specific cases, a defined 90-day phase of increased care density and closer response-guided adjustments.

The Goal

  • To establish a predictable and stable response pattern, allowing a safe transition back to Foundational Care.

Most patients do not require this level of care and this phase is reserved for specific clinical presentations.
Recommendation for the Precision Arc is made based on clinical assessment, not patient preference alone.

The First Step: Comprehensive Diagnostic Evaluation

Every patient journey begins here.

This evaluation is structured around your unique clinical presentation, allowing us to clarify your history, identify relevant patterns, and determine the most appropriate pathway for your care.

Our Clinical Approach

At Harmonia, integrative means approaching psychiatric care with an awareness of the full clinical picture—how symptoms, medications, physiology, and lived experience interact over time.

Many patients come to Harmonia after feeling stalled by fragmented care or medication strategies that were never fully refined for their individual response. Our approach emphasizes thoughtful medication design, guided by clinical reasoning and close attention to how treatments interact and evolve together.

Our ultimate goal isn't to add more; it’s to reach the Minimum Effective Regimen: a clinical state in which treatment is as streamlined as possible while remaining fully effective.

Is Harmonia the Right Fit?

This practice may be a good fit if you:

  • Feel over- or under-medicated, emotionally blunted, or not fully like yourself.

  • Want a careful review of your diagnosis and a clear medication strategy.

  • Appreciate collaborative, well-reasoned treatment decisions where your voice is actually heard.

  • Are seeking a structured, thoughtful approach to medication trials, with the goal of achieving meaningful improvement efficiently.

  • Have tried multiple psychiatric medications without the results you hoped for.

  • Have relied on substances to manage mood or anxiety and want a deeper clinical understanding.

  • Have pursued interventional treatments such as TMS, Spravato, or ketamine with limited or incomplete benefit and are seeking further clinical reassessment.

This practice is likely not the right fit if you:

  • Are seeking "refill-only" visits without periodic review or ongoing clinical oversight. â€‹

  • Prefer rapid medication changes without discussion of rationale, safety, or long-term strategy.

  • Rely exclusively on high-risk medications (such as daily benzodiazepines) without openness to discussing safety, tolerance, or alternatives.

  • Are looking for a one-time prescription rather than a therapeutic relationship.

  • Prefer brief, high-volume visits over a deliberate, personalized approach.

About Nuri Alberga, PMHNP-BC, MBA

Nuri Alberga is a  Psychiatric Mental Health Nurse Practitioner with Advanced training in integrative psychiatry  and extensive experience caring for high-acuity and complex psychiatric cases.

Her approach has been shaped by years of working with medication-sensitive and treatment-resistant presentations—cases where standard approaches proved insufficient.

She brings a high level of clinical judgment to the Precision Arc, focusing on how individual biological variability influences medication response, with the goal of achieving stability for those who have reached a ceiling with traditional care.

Harmonia provides expert psychiatric care for adults with depression, anxiety, ADHD, insomnia, bipolar-spectrum conditions, and co-occurring substance use, including Suboxone/MAT for opioid use, with a particular focus on complex, treatment-resistant, or diagnostically unclear presentations.
